    Elevate Home Health is currently seeking to hire an experienced Registered Nurse (RN) to serve as Director of Patient Care Services (DPCS)!

     

    This position will be based out of our Encinitas office.

    POSITION SUMMARY: The Director of Patient Care Services (DPCS) shall have overall responsibility for coordination of patient care services and shall be responsible for all activities relevant to the patient care services furnished including the development of personnel qualifications and the assignment of personnel.

     


     

    KEY JOB FUNCTION:

    • Supervises staff effectively and completes disciplinary actions as indicated and documents appropriately.
    • Informs the Administrator of any unusual occurrence, accidents or adverse effects of medical devices and documents appropriately and assists with follow-up, as necessary.

    • Participates in establishing community contacts and developing relationships to promote community education and resources to coordinate services and plans and promotes program development.
    • Provides leadership in the assessment of patients and their families and in the development and modification of plans of care as necessary.
    • Monitors utilization of services and ensures services are provided in accordance with applicable regulations and third party payor guidelines.

    Minimum Requirements

    • Must hold a bachelor's degree in nursing.
    • Muse have a current California Registered Nurse (RN) license.
    • Must have at least 2-3 years of experience in a home health care agency.
    • Must have at least 1 year of experience in a supervisory capacity in home health care.
