    Focus Health, a division of Elevate Home Health, is currently seeking a hire a full time weekend home health Registered Nurse (RN) Case Manager to be a part of our fast growing  team!  


    Full time, part time and prn opportunities are available for the general areas of: STOCKTON, LODI, TRACY, MODESTO, &/or  MERCED.


    This position will primarily serve as a "weekend" Registered Nurse (RN) Case Manager, with full time employees working one of the following schedules: 4 days/10 hours Friday - Monday OR 5 days/8 hours Thursday - Monday.

    Minimum Requirements

    • Graduated from an accredited nursing program.
    • Current state license as a Registered Nurse (RN)
    • CPR certified.
    • Must have a minimum of one year clinical practical nursing experience as a Registered Nurse (RN).
    • Must have home health experience.

    Preferred Requirements

    • Homecare Homebase EMR expereince preferred.
